This Employee Assessment of Performance has been written by myself, a Health and Safety Practitioner specialising in the construction industry..

Randomly select an employee then watch them, they do not need to know they are being assessed, complete the form and voila, you have a record of commitment to add to your file. The HSE highly recommends ongoing assessment.
